Hole-in-Head

Is there a doctor in the house? :)
I have a flowerhorn that won't eat and mindlessly drifts around the tank. I notice some pits in its head and it's slowly starving itself. This is the second time I've treated Lucky for this disease.  Sometimes it's alert but it mostly hides at the bottom of the tank. The first time it happened, we removed the old gravel and added new gravel and a rock for a hiding space for a couple of months.  Lucky seemed happy with his new pad but he still got sick.  This is the second bout of the hole in the head and I'm not sure what else to do besides give medication and hope for the best.   I've changed the water and gave it some medicine.  Does anyone know how to prevent this from happening again.

Any advice is greatly appreciated!

change the water more often.

change the water more often. big fish make alot of waste and if he isnt eatting all his food that makes even more waste. changing more water should help also try not feeding at all once a week. he will look for uneaten food and it will help the tank stay cleaner.

I would change it to weekly I

I would change it to weekly I changes at at least 25-30 gallon. After it clears up stop adding epsons salt. Get a bunch of crickets and see if he will eat him, that always seem to perk mine up a bit. Keep feeding the Hikari Gold but add some variety to it with some other foods like Tetra Cichlid and maybe the occasional mealworm.

How big is he?

I forgot to mention it's a 55

I forgot to mention it's a 55 gallon tank. Plenty of room for his size.

Water quality and what you

Water quality and what you are feeding are the causes of hole in the head. Do more water changes and feed him better foods and it should start feeling better. How big a tank, how much water change and what are you feeding?

I change the water every two

I change the water every two weeks religously. I feed him Hikari Gold (Red bag) pellets. After the first epidsode of Hole in the Head, I removed the old gravel and scrubbed the tank, and put new gravel in. He was pretty happy for a few months until now. I add water conditioner, stress coat and epson salt with each water change.

I also notice the top and bottom fin is missing the long tips. He doesn't have fin rot because the fans look good and intact. I keep the tank between 78-80 degrees.

Any suggestion on what kind of food or addictive to boost its immune system?
