You probably have planaria. They're harmless. But, I know they don't look very good. You should probably start doing 20% water changes every week and vaccuuming your gravel at the same time to maintain your tank, rather than every two weeks.
For right now, you should vaccuum your gravel every day and suck up as many of the worms that you can. Also, cut way back on the amount you are feeding your fish. The planaria is a result of over feeding. If you stop feeding your fish, they should start eating the worms.
When you feed your fish don't feed them more than they can eat in 2 minutes. If any of the food touches the bottom, stop feeding them.
Without the excess food in the tank and you vaccuuming them up, they should clear up in about a week. Hopefully, your fish will take to eating them as well. That will help you with the task of getting rid of them.
